Referral partners

Currently health referrals to this scheme are only being accepted from:

• ABC Primary Care Network, Loomer Road Surgery Partnership, Loomer Road, Chesterton, Newcastle under Lyme ST5 7JS

• Hartshill Medical Centre Ashwell Road, Stoke-On-Trent ST4 6AT

We are currently working with other health professionals and primary care networks to increase the number of referring partners to this scheme. But please remember you can refer yourself on to this scheme as well.
