How to join the programme
This is a referral-based scheme so you can only join the programme if you are referred by a GP, School Nurse, social prescriber or health professional.
Referral partners
Currently health referrals to this scheme are only being accepted from:
- ABC Primary Care Network, Loomer Road Surgery Partnership, Loomer Road, Chesterton, Newcastle under Lyme ST5 7JS
- Hartshill Medical Centre Ashwell Road, Stoke-On-Trent ST4 6AT
- Dietetic Department Royal Stoke University Hospital
We are currently working with other health professionals and primary care networks to increase the number of referring partners to this scheme.
